Stereotactic body radiation therapy for intrathoracic oligometastases from endometrial and ovarian cancers: A single institution experience

Background: Stereotactic body radiation therapy (SBRT) is well established for oligometastatic gynecologic malignancies, commonly using 24 Gy in 3 fractions for abdominopelvic lesions. However, outcomes for intrathoracic lesions treated with this regimen are limited. We report institutional outcomes using SBRT 24 Gy in 3 fractions for intrathoracic oligometastases from endometrial and ovarian cancers.

Methods: Patients with oligometastatic endometrial and ovarian malignancies and intrathoracic lesions treated with SBRT (24 Gy in 3 fractions) between 2019 and 2024 were retrospectively reviewed. The primary endpoint was local control (LC). Secondary endpoints included PET response, tumor marker response, overall survival (OS), and toxicity.

Results: The study included 8 patients with 12 lesions, with a median follow-up of 14 months (range: 1–68). Primary sites were ovarian (75%) and endometrial (25%). There was 1 local failure, yielding a 2 year LC of 86%. All evaluable lesions demonstrated PET response, with 86% complete resolution. Tumor markers normalized in all elevated cases. The 2 year OS was 69%. There were no grade ≥2 toxicities.

Conclusions: SBRT 24 Gy in 3 fractions for intrathoracic oligometastases from endometrial and ovarian malignancies appears safe and effective, providing excellent local control with minimal toxicity.
